The Ohio Coverdell Stroke Program, a program within the Ohio Department of Health, is working to assess EMS agencies’ education and quality improvement related to stroke care, as well as the communication between EMS agencies and receiving hospitals. Feedback
from your agency would be helpful in order to inform our work. The survey can be accessed at

1. Stroke education and training
2. Pre-hospital management of stroke patients
3. Stroke data and quality improvement
