The Centers for Medicare & Medicaid Services (CMS) just released the Request for Applications (RFA) for the Emergency Triage, Treat, and Transport (ET3) Model.  Along with outlining essential criteria for participation, the RFA states that ambulance services are eligible for payments at the ALS level under ET3 if Medicare criteria are met.  The RFA also reveals that applicants must provide information about their current compliance programs to participate.    
 
Note: CMS is not accepting applications at this time, but it released the RFA so ambulance services can start preparing today. 
 
Quick Background
The ET3 Model is a voluntary, five-year program that, for the first time, offers Medicare reimbursement for transports to alternative destinations and for treatment in place.  The payment model is designed to improve the quality of care and lower costs to Medicare by reducing avoidable transports to the hospital ED and potentially reducing avoidable inpatient admissions.
 
Who Is Eligible for ET3?
Medicare-enrolled ambulance suppliers and hospital-based ambulance providers may apply to participate in ET3.  Only those selected by CMS will be eligible for reimbursement under the program.  CMS says in the RFA that it aims to select enough participants to capture up to 30% of Medicare FFS emergency ground ambulance transports nationwide
 
The RFA Process
This RFA is the first of up to three potential RFAs through which Applicants may be selected. Additional RFA rounds will be considered based on availability of funding and evidence that the model is working. Here’s the timeline:

Payment Amounts Under ET3
Although CMS initially indicated that it would only reimburse ambulance services at the BLS-E rate for transports to alternate destinations and treatment in place, the RFA states that Participants will be eligible for ALS-E level reimbursement, where applicable criteria are met.  Participants may also get up to a 5% upward adjustment in their payments for treatment in place and transports to alternative destinations.

  • Compliance Programs.  The RFA says “Each Applicant will be required to provide information about current compliance programs and describe plans to ensure compliance with all relevant Medicare and federal fraud and abuse laws.”  We’ll discuss the key elements CMS is looking for in a compliance program.
  • Medical Necessity.  CMS says that medical necessity will apply to transports to alternative destinations.  We’ll cover what CMS will likely look for when making determinations of medical necessity under ET3. 
  • Alternative Destinations.  There are a number of options ranging from urgent care centers to sobriety clinics.  We’ll discuss additional eligibility requirements from the RFA and the agreements you will need.
  • Region Eligibility.  The RFA requires a certain number of Medicare FFS emergency transports in your region.  Find out the requirements and which regions will be given preference. 
  • Telehealth for Treatment in Place.  There are specific requirements and voice only (two-way) radio isn’t enough.  Find out what capabilities you’ll need.
  • Qualified Healthcare Practitioner (QHP).  Treatment in place must involve a QHP.  We will identify who this can be and recommend strategies to fulfill this requirement.
